		<description>Although not involved with the oil and gas industry myself I must concur with Tim that the likely cause was something being done wrong, probably maintenance.

That the time was 6am and shift change makes this more likely rather than less. Picture the scenario: Something breaks during night shift and needs fixing immediately (even if it&#039;s only a temporary bodge job until the day shift arrives). The shift artisan is called out (on a cold weekend night, remember. Was it raining? Sod&#039;s Law says it should on these occasions). He needs to fix the problem in the dark and cold. The shift super is on his back and it&#039;s getting close to knock off time. He&#039;s tired, pissed off, and wants to go home to bed. Hardly the optimium conditions under which to be playing with a welder, cutting torch or angle grinder near high octane fuel tanks. Sure the depot has all sorts of safety procedures etc, but under these circs can any of us put our hands on our hearts and say we wouldn&#039;t cut corners?

This may not be the precise sequence of events, but it is beleivable. I know; there but for the grace of God go I etc etc.

PS, somebody please tell Kerrang Radio that there is no such thing as a &quot;random explosion&quot;. To get an explosion you need to have several conditions just right and do several things just right. Beleive me; I have a couple of certificates in how to make things go bang and a government license that lets me do it.</description>
